If you are downloading an initial out-of-state  (PDF Document)  or out-of-country (PDF Document)  application, you will also need to order a Fingerprint Card.

• All applicants for an initial Kansas license must submit one fingerprint card for the purpose of a KBI and FBI background clearance report. 

• One fingerprint card is also required for applicants whose most recent Kansas certificate/license has expired.

NEW»»Restructured License Fees effective September 1, 2013

The licensure fees set by the state board for FY 2014 streamlines licensure fees into two groups:  $45.00 or $54.00.

(Two exceptions are the Restricted license and the Accomplished license.)
